Netsa Contact + Upload + Zip + Mail is a web based application that will create any type on online forms with the ability to upload attachments, zip them on the server and send the documents and field information onto the desired mail addresses. The app has the following features.

NEED A FORM CREATING , NO PROBLEM , FOR A SMALL $15.00 or £10.00 (IF YOU ARE IN THE UK ) FEE WE CAN CREATE YOUR UNIQUE FORM .

Optimized Ajax/jQuery/PHP. Create any type of online form. Easily design your own styles using the pre defined properties and css. Attach documents and have them zipped up on the server. Send mails in html format. Add Subject and Body to mail message. Assign predefined mail addresses.

Please note the examples on my server are set to 3 files maximum with a maximum combined upload size of 2MB.
